Clinical Management Plans

The file library contains examples of real CMPs, but the DH have also issued two templates which can be used to develop personalised plans.

Template 1 - used when the Supplementary and Independent Prescriber (Doctor or Dentist) have access to a common medical record for the patient.

Template 2 - used when there is no access to a common patient record.
